Helping you and your family reach and maintain good health is very important to Aphase. Aphase is pleased to offer a selection of benefits designed with your health and financial wellbeing in mind. We review our benefit programs each year to ensure we consider the best combination of benefit coverage, network access and affordability for employees. Annual open enrollment is your opportunity to learn about the 2022 Benefits Program, review your current coverage and choose the best options for you and your family.
Here's what you'll need to do to get started:
- View your employee benefits materials in the Benefits Overview for all your benefits program details.
- You can also listen to the open enrollment presentation in the Benefits Overview if you'd like.
- Follow the instructions in the How to Enroll section.
- Make sure you include your completed I-9 form with your enrollment forms.
- Watch the Educational Videos and view the Additional Resources to learn more about your benefits program.
Benefits Overview
Aphase remains committed to providing a competitive, cost-effective benefit program. We are implementing some changes and additions for the 2022 plan year, which we hope will be positively received by you and your families. Below are highlights of changes that will become effective on January 1, 2022. Please review your 2022 Aphase Open Enrollment Employee Benefits Materials for information on all of your benefits.
2022 Benefits Program Updates:
- Priority Health will continue to be our medical/rx insurance carrier.
- It's now required that member ID cards include plan deductibles and out-of-pocket maximum limits, so you can expect to receive a new member ID card from Priority Health with this information. Remember, you can access your digital member ID card any time through your member account at priorityhealth.com or the Priority Health mobile app.
- Health Equity will continue to be our HSA administrator.
- Delta Dental will continue to be our dental insurance carrier.
- We added an enhancement to the dental PPO plan and preventive services will now be covered at 100% (instead of 80%).
- NVA will continue to be our vision insurance carrier.
- Sun Life will continue to be our life and disability insurance carrier.
How to Enroll
Action Required
- You must complete an Aphase enrollment form, even if you are waiving coverage.
- You will need to complete a Sun Life enrollment form if electing voluntary benefits (even if keeping the same).
- Completed election forms must be returned to HR, no later than December 6, 2021 at 5:00 pm.
- You will not be allowed to make a change after the open enrollment window closes, unless you experience a qualifying life event.
Aphase also needs a completed I-9 for legal purposes. Please complete the form and submit along with your benefits forms, even if you are a electing to waive coverage. The form will require your passport info or driver’s license info along with SSN.
